Investigation into Tisdale rail accident now complete

The Transportation Safety Board says a lack of direct supervision and misapplication of safety procedures contributed to the death of a conductor trainee last November.

The accident happened near Tisdale at Murphy’s Interchange Track.

The 39 year old man was struck by a train while attempting to secure a derail mounted between the rails. He died from his injuries in hospital.

The investigation found that when conductor trainees work independently, without direct and close supervision, the risk for an accident occurring increases.  The investigation also found “there is a risk of misapplying safety rules and instructions where there is inadequate reinforcement of proper procedures, reduced training periods, and a lack of continuity and assessment among trainers.”
