Youth In-Transition Worker

The Position
Under the direction of the Well-Being Resource Development Manager, the goal of the Youth In-Transition program is to support youth in their successful transition out of the child welfare system into adulthood. The youth in transition worker helps youth to develop and pursue their goals, and supports them to identify, access, and navigate adult service systems relevant to their specific needs. They support youth to connect to existing supports and resources within their communities including, housing supports, education resources, employment services and training, life skills training (e.g. financial management, household management), health and mental health services, and legal services.
Responsibilities
- Develops relationships with youth in care, building trust with concern and empathy, respecting their confidentiality and privacy, while communicating in a respectful manner.
- Maintains regular communication with MSS case workers to monitor progress of youth in care. Advocates for and promotes well-being for each person in care.
- Identifies and communicates youth needs with the assigned MSS case worker to ensure that the individual case plan is meaningful for the individual, connects with his/her kinship unit and raises awareness of identification with cultural practices and customs.
- Develops relationships with community and shares relevant information through reports and updates on their youth in care, as applicable. Acts as a conduit between community and MSS and manages community expectations.
- Communicates concerns with care or case plans, as needed to the Manager.
- Responds to and de-escalates crisis situations involving youth and provides appropriate interventions for stabilization.
- Communicates regularly with STC staff in various programs and services to facilitate a youth’s access to resources.
- Ensures practices are in accordance with applicable legislation and policies related to child protection or child abuse, special education, attendance, education rights, and privacy that are applicable to youth in care.
- Services are provided to youth in both community-based and drop-in settings which includes after-hours emergency support for youth in service.
- Youth In-Transition workers collaborate with local community agencies to share information about the program as well as to provide knowledgeable referrals for youth to appropriate community services.
- Works closely with Ministry of Social Services to ensure that youth leaving care know about the Youth in Transition program, and also to help navigate the referral process.
- Help eligible youth develop and achieve the goals they have set for themselves through their case plan.
- Support youth leaving care to identify, access and navigate adult service systems relevant to their needs, and transition to adulthood.
- Facilitate access for youth leaving care to existing supports and resources in the community, by providing information and referrals for individual youth that address individual needs and support/reinforce strengths.
- Support youth leaving care to develop and maintain relationships with responsible, caring adults in the community to help them as they transition to adulthood.
- Perform other related duties as defined and assigned by the reporting Director or designate on an as and when required basis.
Education and Experience
- Post-Secondary degree in social work from an accredited post-secondary institution;
- Must have experience in delivering front line child and family services and working with First Nations children, families and communities;
- Must be a member in good standing or eligible for registration with the Saskatchewan Association of Social Work within 6 months of employment.
- Experience working within a governance structure in a community development capacity with First Nations; familiarity with First Nations culture, history is considered an asset.
Knowledge Skills and Abilities
- Must demonstrate the ability to plan, assess, and provide a high standard of delivery of care and services.
- Strong organizational and communications (verbal and written) skills;
- Ability to deal with conflict situations in a professional manner;
- Knowledge of provincial child and family services legislation, regulations, policies and procedures and mitigating factors to ensure actions are legally compliant;
- Ability to apply systematic, logical reasoning when addressing problems or situations in order to arrive at an appropriate solution or outcome;
- Ability to work independently, be self-motivated and driven, and work as part of a multi-disciplinary team.
- Knowledge of Microsoft Excel, Word, Outlook and PowerPoint with an ability to easily learn new software applications.
- Knowledge of the political, social and economic objectives of the Saskatoon Tribal Council and of the Dakota, Cree, and Saulteaux cultures will be considered an asset.
Other Conditions
- Must possess a valid Saskatchewan driver’s license and meet STC’s insurance requirements.
- Must provide a current, original, Canadian Criminal Record Check (CPIC) with vulnerable sector search as a condition of employment.
- Must be willing to work flexible hours, evenings and weekends, and to perform other related duties as required or assigned
