Saskatoon Tribal Council is seeking a highly motivated dedicated individual to fulfil the role of Community Health Nurse for Yellow Quill First Nation (YQFN). The Community Health Nurse will report directly to the Yellow Quill Health Director and will receive clinical supervision and 2nd level Registered Nurse Management support from the Saskatoon Tribal Council. The nurse will be expected to work on site at Yellow Quill First Nation Monday to Friday 8:00 am to 4:30 pm
SUMMARY
The Community Health Nurse will provide community based, culturally relevant community health programming and services as well as support initiatives and strategies that aim to improve health outcomes of community members. The position will work a member of the health team in the delivery of services in community.
The Community Health Nurses are responsible for promoting and protecting health of the community by using knowledge from nursing, social, and public health services. They achieve this by direct patient care as well as by implementing educational programs, recommending policies, administering services, along with conducting research.
DUTIES AND RESPONSIBLITIES:
- Conduct health assessments in community to identify health risks and needs.
- Develop and implement health plans for individuals, families and the community.
- Provide direct health care services to community members within homes, schools and populations.
- Coordinate and manage public health mandatory (immunization and communicable disease) programs
- Provide essential program services of pre- and post-natal care to expectant mothers and newborns.
- Teach individuals and groups about health maintenance and disease prevention.
- Facilitate health-related workshops and seminars to educate community members.
- Advocate on behalf of clients, families and vulnerable populations.
- Refer individuals and families to appropriate health services, social services or other resources as needed.
- Collaborate with other healthcare professionals to provide comprehensive health care services.
- Document patient care and maintain accurate records, ensure patient records and any electronic files are kept secure and maintained in accordance with YQFN policies and procedures and HIPA standards.
- Participate in the development and maintenance of community emergency plan.
- Respond to health emergencies within the community as per community policy.
- Work collaboratively with other health members to support relationships with internal and external programs and agencies.
- Advocate for health policies that benefit the community.
- Participate in accreditation program, maintaining and achieving best practices and quality improvement.
- Provide accurate monthly records relating to program delivery.
- Adhere to the YQFN Personnel policy; and
- Assist and carry out all relief duties and tasks as
QUALIFICATIONS
- Registered Nurse Qualification or Bachelor of Science in Nursing.
- Community Health Nurse Certification and/or previous experience in community nursing setting for 2 years.
- Knowledge of public health principles and practices.
- Registered and in good standing with CRNS.
- Current CPR BLS & First Aide- Health Care Provider.
- Clear CPIC
- Strong interpersonal and communication skills (both oral and written)
- Cultural competency
- Ability to speak Saulteaux and/or previous experience with a First Nation Community an
- Ability to work effectively within holistic team approach; and
- Strong commitment to health prevention, health promotion and community development.
- Working knowledge of PC software applications (MS Office applications, internet, database familiarity).
- Valid Saskatchewan driver’s license.
- Must be willing to work flexible hours and to perform other related duties as required or assigned.
- Willingness to travel extensively, travel subject to variable weather and road conditions
