Public inquest into death of inmate Daniel Tokarchuk

There will be a public inquest into the death of 44-year-old Daniel James Tokarchuk who died on June 7, 2017 in federal custody at the Saskatchewan Penitentiary.

According to a media release, the inquest will be held on Oct. 5 to 9 at the Plaza 88 on Central Avenue in Prince Albert. The first day will start at 10 a.m. with the presiding coroner to decide the start times for each of the following days.

Tokarchuk was found unresponsive in his cell in the early hours of that morning in 2017. Attempts to save his life with CPR and an Automated External Defibrillator were unsuccessful. He was then taken to the Victoria Hospital where he was pronounced dead.

The purpose of an inquest is not a fault-finding mission, but rather a discovery of facts to determine the events leading up to the person’s death. The Saskatchewan Coroners Service is responsible to investigate all unexpected deaths. Coroner Tim Hawyrluk will preside the inquest.
